The Osage Nation Membership Department is responsible for establishing accurate, consistent and permanent records for tribal membership to the Osage Nation and constituency; to serve as custodians of all official records and documents pertaining to membership and CDIB’s; and to ensure that each Osage member has a specific quantum of Osage blood, and/or is a direct descendant from an original allottee.

Membership Card Service


The Osage Nation Membership Department issues membership cards for individuals showing lineal descent from an Osage listed on the base roll created pursuant to the Osage Allotment Act of 1906. Below you will find an application form for applying for membership with the Osage Nation, determination of eligibility, card issuance, etc. Other forms required to process your application for an Osage Membership card may also be found below. Certificate Degree of Indian Blood (CDIB)


The Osage Nation Membership Department works with the Bureau of Indian Affairs (BIA) to process and issue CDIB Cards. Like the Osage Membership card, CDIB’s require verification of lineal descent from an Osage listed on the base roll created pursuant to the Osage Allotment Act of 1906. The Osage Nation Membership Department does not issue CDIB cards and so cannot give a definite turn-around time for receipt of those cards. We do, however, research and process applications and can keep you updated on your status until the point we turn the file over to the BIA. Osage Family Tree Request


Osage Nation population is close to 24,000. The Membership Department will provide any member of the Osage Nation with a copy of their family tree as recorded by the Membership department. These family trees may go back as far as 1906, and in some instances may go back further. For privacy reasons, the Membership department may only furnish members with information concerning their direct lineage. Please complete the Osage Family Tree Request form to start this request. Family Tree requests for minors will only be issued in person, not via email or mail.  Family Trees will not be issued if there is adoption on the lineage.
