Banned Patrons

Patrons of the Osage Casinos may request to be excluded from and not be allowed to enter any and all casinos or gaming facilities owned, operated, or managed by Osage Tribe Gaming Enterprise, d/b/a Osage Casino. You may complete a self-ban form at any of our casinos or you can complete and return the self-ban form below, and return it to the Osage Nation Gaming Commission in-person or via U.S. Mail.

If you have previously completed a self-ban form and now wish to return to casinos or gaming facilities, you may complete the Exclusion Repeal Form below.
