Please make sure you have a chart established at the Wah Zha Zhi Health Center. Items needed for chart are as follows:

1. CDIB/MEMBERSHIP CARD OF FEDERALLY RECOGNIZED TRIBE

2. SOCIAL SECURITY CARD

3. PHOTO ID

4. IF AVAILABLE-MEDICARE, MEDICAID OR PRIVATE INSURANCE
